If you are a social worker, GP practitioner, or someone wanting to make a referral to us in a professional capacity please use the form below. Once we receive your referral we will contact you back to assess the needs of the person needing support to ensure the care we provide is appropriate to their needs. Once this has been assessed, we will arrange to meet with the person in need of support (and yourself if appropriate) so that we can introduce ourselves and discuss a support plan that works for them.
